About the role

As a Registered Nurse - Transplant Care Coordinator, you will play a crucial role in coordinating care for patients undergoing heart transplants, both pediatric and adult. Your responsibilities will include managing patient care plans, ensuring compliance with transplant protocols, and providing emotional support to patients and their families.

Responsibilities

• Develop and implement comprehensive care plans for transplant patients.
• Collaborate with multidisciplinary teams to ensure seamless care coordination.
• Monitor patient progress and adjust care plans as needed.
• Provide emotional support and counseling to patients and families.
• Ensure adherence to transplant protocols and guidelines.
• Participate in ongoing education and training related to transplant care.

Requirements

• Bachelor’s degree in Nursing (BSN).
• Current licensure as a Registered Nurse (RN) in Minnesota.
• Experience in transplant care or acute care setting preferred.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Strong organizational and time management skills.

Qualifications

• Current CPR certification.
• Knowledge of transplant procedures and protocols.
• Experience with electronic health records (EHR).

Pay

• $40.00 - $45.00 per hour based on experience and qualifications.
