About the role

As a Registered Nurse (RN) Transplant Care Coordinator, you will play a critical role in supporting patients undergoing heart and lung transplants. Your responsibilities include coordinating care, managing patient transitions, and ensuring compliance with transplant protocols.

Responsibilities

• Coordinate pre-transplant assessments and evaluations
• Develop and implement care plans
• Manage patient transitions between units and facilities
• Collaborate with multidisciplinary teams to ensure comprehensive care
• Provide education and support to patients and families
• Monitor patient progress and adjust care plans as needed
• Ensure compliance with transplant protocols and regulations

Requirements

• Bachelor’s degree in Nursing or related field
• Current RN license in good standing
• Experience in transplant care or acute care nursing preferred
• Strong organizational and communication skills
• Ability to manage multiple tasks and prioritize effectively
• Proficiency in electronic health record systems

Pay

The salary range for this position is $60,000 - $80,000 annually.
