About the Role

Accountable for the assessment, coordination, delivery, and evaluation of nursing care, including direct patient care, patient/family education, and transitions of care in an acute patient setting. Delivers holistic and individualized care to all patients in an assigned area. Develops, implements, and coordinates a plan of care that incorporates psychological, sociocultural, spiritual, economic, and lifestyle factors. Fosters and maintains collaborative relationships between patients, their family/support group, physicians, and other healthcare providers through timely and effective communications. Adheres to hospital policies, industry standards, best practices, and applicable laws and regulations to promote an optimal patient experience.

Responsibilities

  • Assess, coordinate, deliver, and evaluate nursing care in an acute patient setting.
  • Provide direct patient care, patient/family education, and manage transitions of care.
  • Develop, implement, and coordinate individualized care plans incorporating psychological, sociocultural, spiritual, economic, and lifestyle factors.
  • Maintain collaborative relationships with patients, families, physicians, and healthcare providers.
  • Adhere to hospital policies, industry standards, best practices, and applicable laws and regulations.
  • Communicate effectively with diverse audiences, including non-medical individuals.
  • Prioritize patient care and assignments within standardized policies and procedures.
  • Work independently and as part of a team in a dynamic environment.
  • Engage with team members, patients, and families in stressful situations.
  • Identify and solve routine or complex problems using critical thinking and existing procedures.
  • Ensure the privacy of each patient’s protected health information (PHI).
  • Build collaborative relationships with peers and healthcare providers to achieve objectives.

Requirements

  • Graduate from an accredited school of nursing (Required).
  • BSN (Preferred).
  • Minimum one year current experience in Med/Tele (Required).

Qualifications

  • Registered Nurse (RN) - State Licensure (Required).
  • Basic Life Support Certification (BLS) (Required).
  • Advanced Cardiac Life Support (ACLS) (Required).

Skills and Knowledge

  • Demonstrated knowledge of nursing theories, concepts, and practices; medical terminology; anatomy; and physiology.
  • Working knowledge of the healthcare industry, safety precautions, patient care best practices, and privacy regulations.
  • Strong written and verbal communication skills for explaining sensitive information clearly and professionally to diverse audiences.
  • Time management and organizational skills to prioritize patient care and assignments.
  • Basic knowledge of computer applications, such as Microsoft Office Suite (Word and Outlook).
  • Ability to work independently and as part of a team in a fast-paced environment.
  • Ability to communicate and engage in stressful situations with team members, patients, and families.
  • Critical thinking to identify and solve routine or complex problems.
  • Commitment to ensuring patient privacy and protecting protected health information (PHI).
  • Ability to build collaborative relationships with peers and healthcare providers.
