Qualifications

  • Registered Nurse (RN) license with a minimum of 3 years of healthcare experience, preferably in hospice or related settings
  • Bachelor's degree strongly preferred
  • Leadership or management experience a major plus
  • Valid driver's license (required)

Responsibilities

  • Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and more
  • Ensure adherence to hospice regulations and standards
  • Facilitate communication between medical staff and the care team
  • Lead interdisciplinary meetings, documenting patient needs and care plans
  • Develop and update patient care policies and emergency procedures
  • Manage staff scheduling, performance reviews, and foster a positive work environment
  • Act as the central hub for communication with families, physicians, and other providers
  • Analyze quality metrics and manage a caseload of patients as needed
  • Participate in after-hours and on-call duties to ensure seamless care
  • Maximize resource utilization and promote a collaborative work culture
