Qualifications
- Registered Nurse (RN) license with a minimum of 3 years of healthcare experience, preferably in hospice or related settings
- Bachelor's degree strongly preferred
- Leadership or management experience a major plus
- Valid driver's license (required)
Responsibilities
- Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and more
- Ensure adherence to hospice regulations and standards
- Facilitate communication between medical staff and the care team
- Lead interdisciplinary meetings, documenting patient needs and care plans
- Develop and update patient care policies and emergency procedures
- Manage staff scheduling, performance reviews, and foster a positive work environment
- Act as the central hub for communication with families, physicians, and other providers
- Analyze quality metrics and manage a caseload of patients as needed
- Participate in after-hours and on-call duties to ensure seamless care
- Maximize resource utilization and promote a collaborative work culture
