About the Role
The Swing Bed & MDS Coordinator is responsible for coordinating the Swing Bed Program while ensuring accurate and timely completion of the Minimum Data Set (MDS) assessments in accordance with federal and state regulations. This position oversees resident assessments, care planning, reimbursement documentation, quality measures, and interdisciplinary communication to promote optimal patient outcomes and regulatory compliance.
Responsibilities
- Coordinate all aspects of the Swing Bed Program from admission through discharge.
- Collaborate with physicians, nursing staff, therapy, case management, dietary, pharmacy, and other interdisciplinary team members to develop individualized care plans.
- Verify patient eligibility for swing bed admission according to Medicare and regulatory guidelines.
- Serve as a resource for staff regarding swing bed regulations and documentation requirements.
- Maintain current knowledge of CMS regulations, reimbursement methodologies, and MDS coding updates.
- Collaborate with coding, billing, therapy, nursing, and providers to optimize reimbursement while maintaining compliance.
- Participate in survey readiness activities and assist with corrective action plans as needed.
- Maintain patient confidentiality in accordance with HIPAA regulations.
- Participate in continuing education and professional development.
Requirements
- Associate Degree in Nursing required; Bachelor’s preferred.
- Valid Kentucky RN License.
- Swing bed experience preferred.
- 3-4+ years' experience preferred.
- BLS certification required through American Heart Association (renewals included with employment).
