Job Duties & Responsibilities
- Carries out nursing procedures as ordered by the participant’s physicians following established guidelines.
- Maintains compliance with Federal, State, Licensing regulations and Agency policies and procedures.
- Communicates and collaborates effectively with the participants, families and all other professionals.
- Keeps supervisor abreast of routine and emergent issues involving participants, staff and health related issues.
- Compiles, prepares, and submits reports as requested.
- Complies with all requirements for documentation including but not limited to nursing process, staff related issues and risk management.
- Identifies and reports potential risk management issues related to self, participants, caregivers, visitors, and co-workers and takes appropriate preventive measures.
- Serves as the general health information provider for general health care issues and/or concerns.
- Develops, implements, and evaluates short and long health related plans, goals and objectives.
- Participates in activities that foster personal and professional development.
- Works collaboratively with staff, peers, other health professionals, and volunteers to ensure effective clinical management of Elder Care and practices.
- Provides education and training for employees.
- Performs other duties as assigned.
Program Specific Responsibilities
- Assesses, plans, implements and evaluates the nursing care of participants, including direct care and/or supervision of delegated functions to other members of the nursing team.
- Develops the plan of care and/or collaborative plan of care from admission through discharge with participant, family or caregiver, participant’s and the interdisciplinary team.
- Hires, evaluates, counsels and provides general supervision for all nursing staff.
- Serves as a member of the program interdisciplinary team.
- Represents the Program Director at internal and external meetings as requested.
Education & Experience Requirements
- Graduation from an accredited school of nursing and a minimum of 5 years clinical experience in a geriatric environment that includes (2) years of supervisory/leadership experience or an equivalent combination of education and experience.
- Must possess CPR and First aid certification.
- Must have strong interpersonal skills, well-developed presentation skills.
- Must have the ability to communicate effectively in oral and written form; identify and resolve problems; work effectively with culturally diverse populations.
- Must possess visual and hearing acuity, clear speech, hand coordination and dexterity to perform essential functions of the job effectively.

Physical Requirements & Work Environment
- Repeating motions that may include the wrists, hands, and/or fingers.
- Assessing the accuracy, neatness, and thoroughness of the work assigned.
- Requires the ability to stand 75% of the time and move freely throughout the building including traveling to nearby sites as required.
- Lifting and transferring 110- 200 lbs. (with assistance).
- Regularly pushing wheelchairs.
