About the role

Provide patient care in their homes, following physician prescriptions and adhering to regulations.

Responsibilities

• Develop and update care plans collaboratively with healthcare professionals
• Supervise care provided by aides and nurses, offering instruction, and conducting supervisory visits
• Deliver high-quality clinical services within scope of practice and document visits accurately
• Perform comprehensive assessments, ensuring eligibility, and communicate effectively with patients, families, and team members
• Participate in team activities, orientation, quality improvement, and on-call duties

Qualifications

• Current RN licensure
• Valid driver’s license, vehicle insurance, and CPR certification
• Ability to function effectively in diverse home environments
