About the Role

Provide direct nursing care to patients in the long-term care setting in accordance with established policies, procedures, and protocols of the healthcare organization. Implement and monitor patient care plans, serve as a primary coordinator of all disciplines for well-coordinated patient care, and assure resident safety.

Responsibilities

• Monitor, record, and communicate patient condition as appropriate.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Note and carry out physician and nursing orders.
• Assure resident safety by identifying safety hazards and emergency situations and initiate corrective action as appropriate.
• Assess and coordinate patient's discharge planning needs with members of the healthcare team.

Qualifications

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date (American Heart Association or American Red Cross accepted).
• Registered Nurse obtained prior to hire date or job transfer date required.
• Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
