About the role
The Registered Nurse, or Team Nurse, is a vital member of the clinical team, providing comprehensive primary care for adults 65 and older.
Responsibilities
- Deliver clinical nursing care, patient assessments, education, and support.
- Manage phone and walk‑in triage.
- Oversee anticoagulation activities.
- Assist in developing Health Coaches.
- Perform diagnostic tests (EKG, Spirometry, Hemoglobin A1c, INR).
- Administer medications and vaccines.
- Perform IV initiation, drug therapy management, and wound care.
- Collaborate with multidisciplinary teams to ensure coordinated, patient‑centered care.
- Contribute to process and quality improvement initiatives.
- Requires travel within the area.
Qualifications
- Active RN license in Colorado.
- 2+ years of nursing experience.
