Registered Nurse (RN), Weekend
About the role
As a Registered Nurse (RN), the work you do every day makes a difference in the lives of our patients. Our nurses give patients the ability to spend enhanced quality time with their loved ones in their preferred environment. You will provide one-on-one patient care, work at the top of your license, and utilize leadership skills in coordinating care. You'll be part of an interdisciplinary team focused on compassionate, quality care and producing positive outcomes for your patient population. Home care provides real-world perspective about what helps patients restore their health.
Responsibilities
- Plan of Care: Observe and monitor patient conditions and perform OASIS assessments to develop an individualized care plan and adjust as needs change.
- Minimize Patient Risk: Administer medication as prescribed by the physician and help decrease re-hospitalizations by prioritizing visits for high-risk patients.
- Continuity of Care: Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.
- Family Educator/Advocate: Educate the patient and family on disease processes using teach-back methods to ensure understanding.
- Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Collaborate with the interdisciplinary team and physicians to ensure optimal care is provided.
- Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations. Communicate changes in schedule/availability to schedulers or supervisors.
- Operations: Attend in-service training and mandatory agency meetings.
Requirements
- Registered Nurse with current license in the state of employment.
- Minimum one-year experience as an RN in an acute care setting.
- Home care experience preferred.
- Valid driver's license and auto insurance in your name as a driver.
- Capable of all physical demands of the role.

Schedule
Saturday, Sunday, and 1 weekday, 8am to 5pm.

Dublin, Lewis Center, Plain City, Upper Arlington, Polaris, Alexandria, Blacklick, New Albany, Newark, Sunbury, Utica, Westerville, Hilliard, Grove City, West Jefferson, OSU, Grandview Hts areas.
