About the role

Adaptive Home Health is building a higher‑acuity, patient‑centered, skilled home health model across Texas, with a mission to dramatically improve patient access to home health care. The RN Case Manager is the front line of that mission: delivering skilled care in the home, advocating for patients and families, and helping raise the standard for what home health can be. The role offers autonomy, meaningful patient impact, and a support team built to remove administrative friction. This is a field‑based position serving patients across Texas, supported by technology designed to reduce documentation burden and streamline visit coordination.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high‑acuity home health care, with a focus on wound care and infusion therapy.
• Build and maintain a clear plan of care through consistent communication with physicians, care team members, patients, and families.
