Responsibilities

  • Deliver compassionate clinical care
  • Conduct skilled assessments and provide routine, PRN, admission, discharge, and death visits
  • Provide hospice care that honors physical, emotional, cultural, and spiritual needs
  • Partner with patients, families, caregivers, and interdisciplinary team members to develop and follow a comprehensive care plan
  • Educate families on what to expect, pain management, and comfort measures
  • Manage caseload independently, prioritize calls and visits with urgency, and ensure accurate and timely documentation
  • Serve as a key member of the Interdisciplinary Group (IDG) and communicate effectively with physicians, social workers, aides, chaplains, and referral sources
  • Support the whole person, offering clinical insight and emotional support to families before and after the patient's death, and participate in bereavement planning as needed
  • Perform other duties as assigned

Requirements

  • Registered Nurse (RN) license in the state of Minnesota
  • Valid driver’s license and insured personal vehicle
  • CPR certification (or willingness to obtain within 6 months of hire)
  • Strong clinical assessment, documentation, and time-management skills
  • Willingness to travel to client homes and respond to patient needs during on-call hours
  • Physical capability to perform all essential job functions
  • Ability to read, write, and speak English to ensure effective communication with clients, staff, and families

Pay

$40.00 - $49.00/hour (non-exempt), based on experience, qualifications, and location. Guaranteed pay for 64 hours per pay period.

Schedule

Full-time | 64 hours/pay period | 7 Days On 7 Days Off | 4:30 pm – 8:00 am

Location

Must be available to provide services in our Minneapolis/St Paul territories.
