Hospice Weeknights On-Call Registered Nurse RN

About the role

Responsible for providing compassionate care to patients and families in their homes. Works closely with healthcare providers to coordinate care plans.

Responsibilities

• Provide hospice care services including medication management, symptom control, and emotional support to patients and families.
• Coordinate care with other healthcare professionals to ensure comprehensive patient care.
• Document all interactions and care provided in accordance with hospice standards.
• Attend training sessions and maintain certification in relevant areas.

Requirements

• Bachelor's degree in a related field or equivalent experience.
• Current CPR certification.
• Experience in home health care or hospice preferred.
• Excellent communication and interpersonal skills.

Qualifications

• Valid driver’s license and reliable transportation.
• Ability to work flexible hours including evenings and weekends.
