Registered Nurse (RN) - Virtual / Hybrid

About the role

Exciting opportunity for an experienced RN to work hybrid (remote and on site) as part of a Primary Care team. Upstate Family Health Care Inc. is a Federally Qualified Health Center (FQHC) offering family health care services to individuals of all ages.

Responsibilities

• Monitor electronic medical record system (eCW) for incoming messages, lab results, medication/refill requests, provider notes and other communication
• When needed and as appropriate, respond to patient questions, provide telephonic assessments, document in the EMR (eCW), including with information related to the patient's needs, clinical symptoms, follow established symptom-based protocols and give appropriate clinical advice
• Support the primary care providers in the delivery of health care to patients by performing a variety of activities and procedures which are prescribed by and performed under the direction of the primary care provider(s)
• Provide patient education
• Utilize the nursing process to assess, diagnose, identify outcomes, plan, implement and evaluate an individualized plan of care
• Utilize critical thinking and the nursing process to anticipate and recognize changes in patient status, taking action to modify the plan of care or to elevate to the care team as necessary
• Advocate for patients, uphold their autonomy in decision-making, ensure informed consent is always obtained as warranted or required and assist patients and families in expressing self-determination
• Manage interpersonal relationships for self and with others
• Assume authority and accountability for the nursing care of patients while appropriately delegating or being a delegate of, the elements of care to other members of the care delivery team
• Prioritize and organize time to optimize patient outcomes
• Communicate effectively in a variety of formats in all areas of practice
• Actively partner with others to effect change that produces positive outcomes through the exchange of knowledge
• Promptly answer telephone inquiries regarding caller needs through listening and asking open-ended questions
• Conduct sound nursing assessments and offer alternatives following established nationally-recognized clinical guidelines
• Thoroughly, concisely document all calls with adherence to confidentiality agreements and policy and procedure
• Attend staff meetings, required training, and continued education opportunities
• Maintain current knowledge bases regarding practice issues and actively seek opportunities to enhance same
• Independently seek out guidance/assistance as needed
• Display competency in assessing the health needs of populations across the continuum of life (newborn to 100+ years of age)
• Pend orders, verify precertification and perform other duties as assigned

Qualifications

• Concurrent RN licensure in New York State
• Associate Degree in Nursing Science
• Minimum of two (2) years of clinical experience in a Primary Care practice
• Current CPR certification required

• Hours of operation: Monday - Friday 8:00am - 6:00 pm
• Job Types: Full-time, Part-time, PRN, Permanent, Per diem, Temporary
• Expected hours: 20 – 40 per week
