What You’ll Do As a Home Health RN, You Will Perform comprehensive skilled nursing assessmentsComplete Start of Care, Resumption of Care, Recertification, and Discharge visits as assignedComplete OASIS assessments as requiredDevelop and update individualized plans of careProvide wound care and other skilled nursing treatmentsPerform medication reconciliation and provide medication educationProvide disease-management educationMonitor patients for changes in conditionCommunicate with physicians and obtain necessary ordersHelp identify concerns early and reduce avoidable hospitalizationsEducate patients, families, and caregiversCoordinate care with the interdisciplinary home health teamComplete accurate and timely clinical documentation using WellSky What We’re Looking For Active Maryland Registered Nurse License RequiredGraduate of an accredited nursing programReliable transportationStrong clinical assessment and critical-thinking skillsExcellent communication and documentation abilitiesAbility to work independently in patients’ homesHome Health and OASIS Experience Preferred, but Not RequiredWellSky Experience is a Plus One Patient.
