Registered Nurse (RN), Transplant Coordinator, TACU, Full Time, First Shift

About the role

TACU (Direct Clinical Care) provides specialized outpatient transplant nursing care including IV infusions, lab draws, vascular access, and monitoring of high-acuity post-transplant and rejection-management patients.

Responsibilities

• Engages in population appropriate communication.
• Has knowledge of growth and development milestones and tasks.
• Gives clear instructions to patients/family regarding treatment.
• Involves family/guardian in the assessment, initial treatment and continuing care of the patient.
• Identifies any physical limitations of the patient and deploys intervention when necessary.
• Recognizes and responds appropriately to patients/families with behavioral health problems.
• Interprets population related data and plans care appropriately.
• Identifies and responds appropriately to different needs resulting from unique psychological needs or those associated with religious / cultural norms.
• Performs treatments, administers medication or operates equipment safely.
• Recognizes and responds to signs/symptoms of abuse or neglect.
• Carefully coordinates and manages patient care activities across the transplant continuum (evaluation, waitlist, inpatient, and post-transplant).
• Attends and participates in weekly transplant clinics to assess patients in person, manage complex medical needs, and collaborate with providers in real time.
• Performs and interprets clinical assessments, review test results, and escalates abnormal findings to the provider.
• Maintains accurate and timely documentation in the EMR and transplant databases (UNOS, Phoenix, SpectraPath, NKR, etc.) ensuring compliance with institutional, CMS, and UNOS requirements for data accuracy, timeliness, and completeness.
• Supports the development and revision of clinical policies and pathways.

Qualifications

• Minimum required: Associate degree.
• Preferred: Bachelor's Degree.
• Required: Current RN license in the State of Ohio.
• Alexander’s Life Institute (ACLS) preferred.
• A minimum of three to five years of clinical nursing experience required, with at least three years in transplant, nephrology, hepatology, critical care, or complex medical-surgical nursing preferred.

Pay

Salary range: $50,000 - $70,000 annually.
