Registered Nurse (RN) - Transitional Care Unit

About the role

Registered Nurse (RN) in the Transitional Care Unit, providing direct patient care in a fast-paced, multidisciplinary environment. Care for diverse patients with complex medical conditions at a Level I Trauma Center; gain experience in a dynamic setting that fosters clinical growth and prepares for specialty nursing roles, especially in emergency care.

Responsibilities

• Assess, diagnose, plan, implement, and evaluate patient care
• Develop and communicate individualized care plans
• Educate patients and families on disease management, medication, and post-discharge needs
• Promote patient safety, quality, and regulatory compliance
• Collaborate with healthcare professionals to ensure optimal outcomes
• Evaluate physical, psychosocial, and emotional needs, considering patient preferences and barriers

Qualifications

• Nursing diploma or associate degree required
• RN license essential
• Bachelor’s degree and less than 2 years of experience preferred
