Registered Nurse (RN) - Support Nurse-Onsite

About the Role

The Nurse position will facilitate Reserve Care's efforts to relieve suffering, enhance comfort, promote quality of life, and foster choice in the presence of an advanced serious illness for all patients and families. The practice is based on competent clinical knowledge and focuses on clinical practice, education, and collaboration related to palliative and hospice care.

Responsibilities

Clinical Practice

• Uses the nursing process to gather information, assess (RN), plan, implement, and evaluate patient care to provide for the unique physical and emotional needs of each patient, including providing handouts and videos.
• Establishes and maintains communication and collaboration with the interdisciplinary team and patient’s primary physician regarding patient condition and plan of care.
• Anticipates needs to ensure exceptional patient-centered care.
• Completes phone follow-up with patients who have called in over the weekend or after hours.
• Handles incoming nursing-based calls from the care solutions center throughout the day.
• Follows up with patients who have been discharged from the hospital, noting medication changes, increased care needs, and/or changes in condition, and verifies or adjusts nurse visit schedules accordingly.
• Communicates referral criteria based on physical symptoms, functional/cognitive changes with patients/families, and refers to hospice level of care when needed.
• Coordinates care within the agency, between transdisciplinary team members, and with external partners.
• Ensures documentation is consistent with Reserve Care’s guidelines, painting a clear picture of conversations and outcomes.
• Contributes to and follows the plan of care in alignment with patient/family goals, collaborating with the primary nurse, social worker (SW), HNAS, SCC, and other team members.
• Complies with program practices and guidelines in all interactions with team members, patients, or families.
• Performs chart reviews to determine the need for sooner visits by the primary nurse and maintains a spreadsheet with assigned information.
• Makes phone calls to patients/families per the team’s direction for follow-ups on medication changes, address changes, training needs, etc.
• May participate in transdisciplinary team conferences, offering insight on patients discussed or those needing discussion based on phone support.

Education

• Educates and trains patients and families on applicable areas of hospice care, such as interventions for symptom management within the scope of practice.
• Empowers patients and families to identify goals of care.

Requirements

• Maintains a current Registered Nurse license in the state of Ohio.
• Minimum of two years’ experience in hospice and palliative care preferred.
• Must be flexible and have problem-solving skills, prioritize tasks, and demonstrate healthy coping strategies.
• Ability to demonstrate excellence in written and oral communication.
• Knowledge of hospice and palliative care clinical standards of practice, policies, and procedures.
• Ability to practice in accordance with Reserve Care’s clinical standards of practice, policies, and procedures.
• Ability to establish and maintain collaborative working relationships with Reserve Care employees, the community, and healthcare providers.
• Compliance with Annual Flu Shot Policy or ability to provide exemption documentation.
• Ability to provide proof of eligibility to work in the United States.
• High level of integrity, ethics, and professionalism.
