About the role

We are seeking a Registered Nurse (RN) who is caring, attentive, and committed to high-quality clinical care. The RN will assess, plan, implement, and evaluate nursing care for our residents, ensuring their physical and emotional needs are met in a safe and supportive environment.

Key Responsibilities

  • Provide direct nursing care to residents in accordance with care plans, standards, and facility policies.
  • Conduct thorough resident assessments and document findings accurately.
  • Develop, implement, and revise individualized nursing care plans.
  • Administer medications and treatments safely and according to orders.
  • Monitor and record changes in resident condition; promptly communicate significant changes to the care team and providers.
  • Collaborate with physicians, nursing staff, rehabilitation therapists, social workers, dieticians, and families.
  • Supervise and support licensed practical nurses (LPNs), certified nursing assistants (CNAs), and other care staff.
  • Ensure compliance with regulatory standards and participate in quality improvement activities.
  • Educate residents and families on care needs, medications, and safety practices.
  • Assist with admissions, transfers, and discharge planning.
  • Promote a therapeutic, respectful, and caring environment for residents and families.

Qualifications

  • Current and valid RN license in the state of New York.
  • CPR/BLS certification.
  • Prior experience in long-term care, rehabilitation, or skilled nursing preferred.
  • Strong clinical assessment, critical thinking, and communication skills.
  • Preferred: Experience with electronic medical records (EMR).
  • Demonstrated ability to lead and mentor clinical team members.
  • Commitment to resident-centered care and teamwork.
