Registered Nurse (RN) - Senior LIFE Indiana

About the role

Our RNs work as an extension of our Clinic Team but with the autonomy and flexibility of providing that care in the home and community settings. We serve the senior population of Indiana and a limited surrounding area so you won’t spend all of your time in a car. You will be doing what you do best in providing the care to enable our participants to live their best lives exactly where they most want to be - at home!

Responsibilities

• Provide high-quality, compassionate care to seniors in their homes and communities.
• Assess patient needs and collaborate with the Senior LIFE team to develop and implement individualized care plans.
• Administer medications as prescribed and monitor patient responses.
• Document patient care activities accurately and maintain thorough medical records.
• Communicate effectively with patients, families, and other healthcare providers to ensure continuity of care.
• Ensure compliance with all regulatory requirements and safety protocols.

Requirements

• Current Registered Nurse license in the state of Indiana.
• At least 1 year of recent experience in home health or related field.
• Strong communication and interpersonal skills.
• Ability to work independently and manage multiple tasks.
• Valid driver’s license and reliable transportation.

Qualifications

• Experience working with the senior population.
• Knowledge of home health care procedures and regulations.
• Basic computer skills for accessing patient records and documentation.
