Registered Nurse (RN) - PRN

Field-based, part-time role with a schedule based on availability and a minimum of 10 points per week.

Responsibilities

• Deliver excellent home health care to patients in Detroit, setting the clinical bar, documentation standard, and referral-source trust.
• Carry a high-quality caseload, ensuring visit quality and OASIS accuracy that holds up to review.
• Own your patients' outcomes by driving low avoidable-hospitalization rates and strong patient-experience scores.
• Participate in case conferences and team huddles to improve quality.
• Act as a referral-source ambassador, building trust with discharge planners, ALFs, and physicians.
• Set the standard for charting, cadence, and judgment as a template for onboarding future cohorts.
• Perform comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).
• Provide direct, high-acuity patient care with a focus on wound care.
• Build and manage the plan of care with physicians; supervise LPNs and HHAs on your cases.
• Represent Adaptive to patients, families, and referral sources.

Requirements

• Active Michigan RN license.
• Reliable transportation, valid driver's license, and auto insurance.
• Home health and OASIS experience strongly preferred.

Pay

$70.50 per point, with mileage reimbursement in addition to productivity compensation.
