Responsibilities

  • Deliver excellent home health care to patients in Detroit, setting the clinical bar, documentation standard, and referral-source trust.
  • Carry a high-quality caseload: ensure visit quality with OASIS accuracy that holds up to review.
  • Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve quality.
  • Be a referral-source ambassador: build trust with discharge planners, ALFs, and physicians to grow referrals.
  • Set the standard others copy: your charting, cadence, and judgment become the template for onboarding future cohorts.
  • Perform comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).
  • Provide direct, high-acuity patient care with a focus on wound care.
  • Build and manage the plan of care with physicians; supervise LPNs and HHAs on your cases.
  • Represent Adaptive to patients, families, and referral sources.

Requirements

  • Active Michigan RN license.
  • Reliable transportation, valid driver’s license, and auto insurance.
  • Home health/OASIS experience strongly preferred.
