About the role
Works under the supervision of the Nurse Manager and the Director and is responsible for the assessment, planning, implementing, and evaluating the total nursing care of assigned patients. The RN utilizes communication, critical thinking, problem solving, and decision making to create a care plan for each patient based on physiological, psychosocial and cultural diversity.
Responsibilities
- Demonstrates ability to assess patient clinical and psychosocial needs by applying standards of practice as it relates to the specialty or patient population.
- Demonstrates nursing unit specific competencies and equipment training, see checklists.
- PARTNERS WITH PATIENTS AND THEIR FAMILY TO DEVELOP, IMPLEMENT AND EVALUATE A PLAN OF CARE, TREATMENTS AND PROCEDURES THAT CONTRIBUTE TO PHYSIOLOGICAL AND PSYCHOSOCIAL OUTCOMES OF THE PATIENT.
- Incorporates evidence based standards of practice and collaborates with interdisciplinary team in care coordination.
- Facilitates safe patient transition to next level of care as part of plan of care.
- Focuses on quality outcomes of patient when addressing service needs to ensure evidence based quality care of patient.
- MODIFIES EDUCATION METHOD TO ENSURE PATIENT/FAMILY UNDERSTANDING.
- ASSUMES RESPONSIBILITY TO COORDINATE AND DELIVER INDIVIDUALIZED PATIENT EDUCATION AND DISCHARGE SPECIFIC EDUCATION TO FAMILY/CAREGIVERS. EVALUATES FOR COMPREHENSION AND DOCUMENTS.
- EVALUATES RESOURCES AND SUPPORT NEEDED AND USES SYSTEMS TO ENSURE NEEDS ARE MET.
Requirements
- Bachelors Degree, Nursing (Preferred)
- Associates Degree, Nursing (Preferred)
- Program Graduate, Nursing (Nursing Diploma is minimum education requirement, Associate’s or Bachelor’s in Nursing accepted in lieu of Nursing Diploma)
- High School Diploma or GED, HS Diploma/GED only required if Nursing Diploma Graduate
- CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) Upon Hire
Qualifications
- Registered Nurse (RN) - Virginia Department of Health Professions (VDHP) or licensed in a compact state Upon Hire
