Registered Nurse (RN) - On-Call Community Hospice

Full-time, night shift position providing on-call hospice care in the Capital Region and Saratoga County areas.

About the Role

The Community Hospice is seeking a compassionate and dedicated Full-Time Registered Nurse (RN) to join our After-Hours Hospice Care Team. In this role, you will provide critical nursing care and support to hospice patients and families in their homes and nursing facilities during overnight hours. You'll make a meaningful difference by offering expert symptom management, comfort-focused care, and compassionate end-of-life support.

• Help patients remain comfortable and supported in their preferred care setting.
• Work alongside physicians, nurses, social workers, chaplains, hospice aides, complementary therapists, and volunteers in a supportive interdisciplinary team environment.
• Receive national pre-taxed mileage reimbursement.

Responsibilities

• Provide after-hours nursing visits to hospice patients in home and facility settings.
• Assess and manage symptoms, addressing urgent patient and family needs.
• Provide education, guidance, and emotional support to patients and caregivers.
• Coordinate care with the interdisciplinary hospice team.
• Communicate changes in patient condition and ensure appropriate follow-up.
• Maintain accurate and timely clinical documentation.

Requirements

• Current New York State Registered Nurse (RN) license.
• ASN required; BSN preferred.
• 1 year of RN experience required.
• Valid driver’s license and reliable transportation.
• Strong assessment, communication, and critical thinking skills.

Preferred Qualifications

• One to two years of nursing experience in home care.
• Previous hospice or palliative care experience (preferred but not required).

No hospice experience? We welcome RNs passionate about compassionate care and eager to learn. Comprehensive orientation and training programs are provided. We also offer ride-along opportunities with experienced nurse preceptors to explore hospice nursing firsthand.

Pay

Pay range: $38.00–$50.18 per hour, based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The range may vary based on location.
