Territory/Area of Coverage
Portsmouth, Exeter, Durham, Farmington, Milton, Kingston, Brentwood, Wolfeboro, Dover, Rye, Rochester
Responsibilities
- Be the Lifeline: Respond to a variety of patient needs, from urgent to routine, providing unwavering support in critical moments.
- Provide Empathetic and Heartfelt Assessments: Conduct comprehensive assessments of patients' physical, emotional, social, and spiritual needs to truly connect with them.
- Maintain Accurate Documentation: Ensure every detail is recorded to deliver high-quality care, both in the patient's home and within our agency and regulatory standards.
- Provide Support for Families: Share your knowledge and expertise to guide and support families in caring for their terminally ill loved ones, offering confidence and reassurance during this challenging time.
Requirements
- Bachelor's degree in nursing or healthcare preferred.
- Current unencumbered license as a Registered Nurse (RN).
- Minimum one (1) year of experience as an RN in a medical-surgical/acute care setting.
- Hospice experience is a plus.
- Computer proficiency, including experience with EMR (Electronic Medical Records).
- Current CPR certification if required by state.
Pay
$80,000 – $90,000 per year (average pay range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
