Registered Nurse (RN), On Call 7 On 7 Off Hospice

About the role

Phoenix Home Care & Hospice is seeking an experienced and compassionate On-Call Registered Nurse to support their Topeka hospice team. This role involves responding to immediate care needs, completing routine and as-needed visits, triaging urgent situations, collaborating with physicians, and working closely with the interdisciplinary team.

Responsibilities

• Respond to immediate care needs of hospice patients and families during nights and weekends
• Complete routine and as-needed visits in home settings across the Topeka region
• Triage urgent situations and collaborate with hospice physicians to provide comfort and symptom management
• Work closely with the interdisciplinary hospice team, community partners, and family caregivers
• Maintain accurate clinical documentation and uphold all regulatory and agency standards
• Deliver professional, compassionate, patient-centered hospice care
• Participate in ongoing training, education, and quality initiatives

Requirements

• Kansas RN License or Compact License
• Hospice nursing experience preferred (Topeka hospice experience is a plus)
• Strong clinical judgment with the ability to work independently in home settings
• Excellent communication and customer service skills
• Valid driver's license and reliable transportation
• High-level organization, time management, and emotional resilience

$75,000 - $95,000
