About the role

The On-Call Hospice RN responds to all Hospice patients and their families, as well as handles referrals or community inquiries outside of office hours every other weekend. This role provides urgent clinical and psychosocial interventions, skilled pain and symptom management, and collaborates with providers within and outside the hospice team to maximize positive patient outcomes. The position requires independent assessment and judgment in crisis situations, with consultation available from the Hospice Medical Director and administrator on call.

Responsibilities

  • Ensure consistent availability by telephone and email during assigned shifts.
  • Conduct urgent physical and psychosocial assessments of patients and families during after-hours and determine appropriate interventions.
  • Provide and maintain a safe environment for the patient.
  • Deliver professional nursing care using all elements of the nursing process.
  • Focus on skilled nursing care, particularly pain control and symptom management.
  • Offer interim guidance to patients and families regarding diagnosis, medications, treatments, and progress.
  • Document patient care thoroughly and timely, including clinical notes, pain/symptom management changes, and outcomes.
  • Communicate and collaborate with physicians regarding patient needs, report changes in condition, and obtain/receive physician orders as required.
  • Work closely with members of the Interdisciplinary Group (IDG) to ensure continuity of care and support for patients and families.
  • Collaborate with vendors to obtain durable medical equipment (DME), supplies, and medications after hours.
  • Coordinate with hospital and community residential staff to adjust interventions and plans of care as needed.
  • Conduct after-hours hospice admissions as necessary.
  • Participate in mandatory meetings as required.
  • Pursue personal growth and development, maintaining and upgrading professional knowledge through continuing education and in-services.
  • Fulfill obligations of requested assignments and perform other job-related duties as assigned.

Requirements

  • Commitment to the hospice philosophy of care and ability to function effectively as a team member.
  • Comprehensive knowledge of medications, treatments, and therapies for pain and symptom management.
  • Ability to visit patients wherever they reside.
  • Consult with physicians, team members, and administrative staff as needed.
  • Provide calm, courteous, and professional care to individuals from diverse backgrounds.
  • Maintain client confidentiality in accordance with HIPAA laws and company policy.
  • Graduate of an accredited school of nursing.
  • Current unencumbered license to practice as a Registered Nurse in Florida.
  • Preferably two years of experience in hospice, home care, hospital medical-surgical, or oncology nursing.
  • Understanding of state and federal laws, regulations, and nursing scope of practice.
  • Ability to complete documentation accurately and in a timely manner.
  • Valid driver’s license, insured automobile in good working order, and ability to provide own transportation.
  • Satisfactory references and successful criminal background check (contingent upon job offer).

  • Physical ability to perform prolonged standing, walking, lifting, positioning, and transferring patients.
  • Capability to lift supplies and equipment, and perform tasks requiring reaching, stooping, bending, kneeling, or crouching.
  • Visual acuity and hearing sufficient to perform required nursing skills.
