Registered Nurse (RN) - Neuro/Tele-Medicine

About the Role

Performs an age-specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation, and evaluation of patient care. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills and clinical judgment in coordinating patient care and directing/delegating activities of the patient care unit team.

Responsibilities

• Conducts patient interviews, explains policies and procedures, reviews patient’s chart, and answers questions; assesses gastrointestinal, cardiovascular, respiratory, renal, and neurological health status; determines mobility, sensory deficits, prostheses use, and skin condition, adjusting the plan of care as needed.
• Assesses level of pain and pain management; communicates and documents patient’s physiologic health status and plan of care.
• Evaluates patient’s psychosocial health status; identifies coping mechanisms, knowledge level, cultural/ethnic requirements, and expectations of care; communicates and documents psychological status and care plan.
• Identifies patient outcomes, develops criteria for measurement, and establishes individualized goals; collaborates with physicians and health team members to implement the plan of care.
• Implements the plan of care, ensuring safety precautions, administering treatments, using equipment, and providing emotional support; reassesses patients and revises the plan as needed.
• Documents patient’s treatment, activity, condition, and responses to interventions; uses appropriate methods of documentation per departmental policy.
• Acts rapidly during emergencies, manages self and others, and maintains a calm environment; participates in discharge planning and provides instructions to patients and families.
• Evaluates care effectiveness, performs variance analysis, and improves systems for patient care; collaborates with care team members and physicians on treatment regimens.
• Accurately interprets and implements physician-prescribed treatments; assists during procedures and communicates changes in patient condition.
• Performs decubitus care (grade I-IV), preventive skin measures, dressings, compresses, irrigations, and other skin-related procedures.
• Administers medications safely, educates patients on drug/food interactions, and documents administration correctly.
• Provides patient and family education, explains conditions, and evaluates teaching effectiveness through feedback.

Requirements

• Graduate from an accredited School of Nursing; Bachelor’s Degree in Nursing preferred.
• Must enroll in an accredited BSN program within two years and obtain a BSN Degree within five years of job entry.
• Current license to practice as a Registered Professional Nurse in New York State.
• Specialized certifications as needed.

The salary range and/or hourly rate is a good-faith determination of potential base compensation. Factors considered include location, specialty, years of experience, education, credentials, and internal equity.
