Community Hospice has served the Capital Region for decades, and we're looking for an RN leader ready to make an immediate impact on patient care in Albany and the surrounding communities. This is your chance to lead a high-performing Hospice Intake and Admissions team at an organization that's investing in growth, innovation, and the people who make hospice care possible.

Responsibilities

  • Provide administrative direction, clinical oversight, and daily operational leadership for Intake and Admissions across the Hospice service area.
  • Build and reinforce a culture where speed and responsiveness to referrals is critical, providing support and assistance to ensure an easy, fast intake process.
  • Ensure rapid turnaround for admissions, recognizing that patients are often referred late and time is a key factor in care outcomes.
  • Arrange timely admissions, ensuring accessibility 365 days a year.
  • Partner collaboratively with Provider Relations on the education of healthcare professionals, referral sources, and the general public regarding hospice care and eligibility.
  • Ensure compliance with regulations, accreditation standards, and agency policies and processes.
  • Serve as a member of the Community Hospice senior leadership team, helping shape strategy for the organization's future.

Requirements

  • A current license to practice as a Registered Nurse in the State of New York.
  • BSN preferred but not required.
  • Prior supervisory experience strongly preferred.
  • Minimum of three (3) years hospice, acute care, or homecare RN experience required.
  • Comfort providing direct oversight of the intake department and stepping in to complete a hospice admission when needed.
  • Prior hospice experience preferred.

Qualifications

  • Competitive pay: $48.00–$63.38/hour, based on experience, skills, and education.
  • Exempt positions under the Fair Labor Standards Act (FLSA) are paid within the base salary equivalent of the stated hourly rates.
  • The pay range may vary within the stated range based on location.

Pay
