Registered Nurse (RN) specializing in Long Term Acute Care, providing critical inpatient post-ICU services for chronically ill patients.
Responsibilities
- Collaborate with physicians and preceptors to develop, implement, and evaluate individualized care plans.
- Conduct comprehensive assessments of patients and families, interpret data to identify problems, and establish goals.
- Participate actively in interdisciplinary team efforts to meet rehabilitation objectives.
- Ensure accurate, complete, and timely documentation in medical records.
- Assess patient needs, develop and implement care strategies, contribute to team assessments and planning, and maintain thorough documentation.
- Support patient recovery during vulnerable moments with compassionate care.
Qualifications
- Graduate from an accredited nursing program.
- Valid RN license.
- BLS certification.
- Flexibility in scheduling is required to meet program demands.
