Registered Nurse RN in SNF

About the Role

As a registered nurse, you will make a difference in the lives of our residents, caring for them with passion and dedication, building meaningful relationships along the way. You will perform and supervise day-to-day nursing activities in the provision of direct resident care within the scope of your licensure and in accordance with federal, state, and local guidelines, as well as facility policies and procedures.

Responsibilities

• Perform resident assessment through direct physical assessment, behavior monitoring, various assessment forms, review of hospital records, laboratory results, resident medical records, and information provided by the interdisciplinary team, resident, and/or the resident's significant others.
• Monitor residents for changes in condition that may indicate progress or deterioration in the resident's overall status. Notify physicians and families of changes and transcribe orders received.
• Administer medications according to facility policy and monitor residents for therapeutic and adverse effects. Notify physician and pharmacy of suspected and actual adverse reactions. Document medications on the Electronic Medication Administration Record (EMR) immediately when given or refused.
• Provide various treatments as ordered by the physician, such as application of topical ointments/solutions, dressings, and respiratory treatments. Document treatments on the EMR Treatment Administration Record immediately when completed.
• Triage emergency situations and provide appropriate first aid in a timely manner. Complete appropriate reports and consult with the Assistant Director of Nursing (ADON), Unit Manager, or charge nurse on duty.
• Collaborate with physicians to assure timely visits, follow-up visits as indicated, signing of physician orders, and progress notes per policy and regulation.
• Supervise Certified Nursing Assistants (C.N.A.'s) on their shift to assure proper and timely delivery of resident care and completion of assignments. Complete work assignments with C.N.A.'s and communicate any changes in the resident care plan or condition of the resident.
• Maintain Protected Health Information (PHI) by ensuring C.N.A.'s adhere to facility policy and procedure.
• Understand, comply with, and promote all rules regarding resident rights. Educate and ensure C.N.A. compliance with respect to resident rights. Report all suspected, alleged, or witnessed physical, sexual, or verbal abuse immediately to the Administrator, Abuse Prevention Coordinator, and/or designee.
• Attend all staff meetings and in-services as required. Participate in organization-wide programs and meetings to stay informed and current on organizational strategies and programs.
• Maintain competence with specified nursing skills by performing competency training as directed by the Director of Nursing (DON).

Requirements

• Current State of Illinois RN licensure.
• Current CPR card.
• Graduate of an accredited Registered Nursing program.
• One-year experience in long-term care nursing preferred.

Schedule

• Full-time positions available: 3:00 PM – 11:00 PM and 11:00 PM – 7:00 AM.
