Registered Nurse (RN) - Hospital Care Manager, PRN, Days

About the Role

Serves as a patient engagement and care transition specialist, collaborating with physicians and multidisciplinary teams to ensure efficient, high-quality, and cost-effective care. Focuses on maximizing patient autonomy and aligning long-term care plans with patient and caregiver goals.

Responsibilities

• Partner with healthcare providers to optimize resource utilization and promote effective communication during hospitalization.
• Identify social determinants of health, coordinate post-acute care plans, and facilitate patient access to resources.
• Address system barriers, drive performance improvements, and ensure compliance with regulatory standards.
• Assess psychosocial risks, maintain clinical knowledge, and ensure timely and appropriate care planning.
• Navigate regulatory requirements, communicate with payors, and advocate for patient rights and independence.

Requirements

• Bachelor's degree in Nursing or equivalent; RN licensure in South Carolina.
• Minimum of three years acute care nursing experience; case management experience preferred.
• Certifications such as ACM or CCM are preferred.
• Knowledge of medical necessity criteria and care management standards.

Schedule

Day shifts.

This non-management role reports to a supervisor and emphasizes compliance, patient advocacy, and care coordination to improve healthcare outcomes.
