About the Role

This position serves as a care team partner focused on patient engagement, care transition, and aligning long-term care plans.

Responsibilities

• Collaborate with physicians and multidisciplinary teams to ensure efficient, quality, and cost-effective care.
• Address social determinants of health, resolve system barriers, and facilitate post-acute care coordination.
• Advocate for patient rights and independence, navigate regulatory and insurance requirements.
• Maintain expert knowledge of clinical outcomes and standards.
• Promote patient autonomy and access to resources while ensuring compliance with care management standards.

Requirements

• Bachelor's degree in Nursing with three years of acute care nursing experience.
• Case management experience preferred.
• Licensure as a Registered Nurse in South Carolina required.
• CCM or ACM certification preferred.
• Knowledge of medical necessity criteria is a plus.
