Registered Nurse (RN) - Hospital Care Manager, Part-Time, Days

Role Overview: This position is a care management partner responsible for patient engagement, care transition, and aligning long-term care plans. Key Responsibilities Collaborate with physicians and multidisciplinary teams to ensure efficient, quality, and cost-effective care delivery.Facilitate communication and resource access, addressing social determinants of health and post-acute care needs.Identify and resolve system barriers impacting diagnosis or treatment, promoting performance improvements.Navigate value-based care with a focus on patient-centered, goal-aligned plans.Assess psychosocial risks, maintain clinical knowledge, and ensure compliance with regulatory and insurance requirements.Advocate for patient empowerment, rights, and access to necessary services, including handling care denials and regulatory documentation.Coordinate with payors and ensure timely, appropriate care plans in accordance with hospital policies. Qualifications & Skills Bachelor's degree in Nursing or equivalent with current RN license.At least three years of acute care nursing experience; case management experience preferred.Optional: diploma or Associate degree with agreement to obtain a BSN within four years.Certifications like ACM or CCM are preferred.Knowledge of medical necessity criteria is advantageous.
