Registered Nurse (RN) - Hospital Care Manager, Part-Time, Days

Role Overview: This position serves as a key team partner focused on patient engagement and care transition from acute settings, collaborating with physicians, clinical staff, and payors to ensure high-quality, cost-effective care. The role involves leading multidisciplinary teams to maximize patient autonomy and aligning long-term care plans. Key Responsibilities Promote effective utilization of clinical resources and coordinate post-acute care with the interdisciplinary team.Identify social determinants impacting health outcomes, facilitate resource access, and address system barriers.Assess psychosocial risks, maintain expertise in clinical outcomes, and ensure compliance with regulatory standards.Navigate complex insurance and regulatory requirements, advocate for patient rights, and empower families in healthcare decisions.Ensure timely care plans, appropriate care levels, and medical necessity in accordance with hospital policies. Qualifications & Skills Bachelor's degree in Nursing or equivalent, with three years of acute care nursing experience; case management experience preferred.Valid RN license recognized in the practice state; certifications such as ACM or CCM are preferred.Knowledge of medical necessity criteria and regulatory requirements.
