Registered Nurse (RN) – Hospice Field Case Manager | East Pasco

About the role

Gulfside Healthcare Services is seeking a compassionate Registered Nurse (RN) to provide hospice field care throughout East Pasco.

Responsibilities

• Support patients and families in private residences, assisted living facilities, and skilled nursing facilities
• Provide comfort-focused care, education, and support during the end-of-life journey
• Manage an assigned patient caseload
• Develop individualized care plans
• Collaborate with the interdisciplinary team
• Provide guidance to LPNs, CNAs, and other members of the care team

Qualifications

• Active Florida Registered Nurse (RN) license or Compact State RN license required
• Minimum of one (1) year nursing experience preferred
• Hospice, home health, case management, or acute care experience preferred
• Strong assessment, communication, critical thinking, and time management skills
• Career-oriented, organized, and able to work independently while supporting a team-based approach to care
• Valid Florida driver’s license, reliable transportation, and proof of automobile insurance required
• Proficiency with Electronic Medical Records (EMR) preferred
