About the role

Join our hospice team serving the coastal region out of Rockport and help patients and families navigate one of life's most important journeys with compassion, dignity, and expert care.

Responsibilities

  • Provide skilled nursing care and coordinate services for terminally ill patients in the comfort of their own homes.
  • Build meaningful connections with patients and families, serving as a trusted guide, advocate, and source of support during a deeply personal time.
  • Develop and coordinate individualized care plans that support comfort, quality of life, and patient goals.
  • Work closely with an interdisciplinary team to ensure comprehensive care for patients.
  • Manage your day, start and end your work from home, and focus on providing one-on-one care that truly makes a difference.

Requirements

  • Graduate of an accredited School of Nursing required; BSN preferred.
  • Current applicable state(s) license as a Registered Professional Nurse required.
  • Current BLS certification required or must obtain within 30 days of start date.
  • Valid Driver’s License to operate a motor vehicle, with vehicle available for work covered by liability insurance as specified by agency policy required.
  • One year of recent RN experience required.

Qualifications

  • Automatic advancement from RN I level if all onboarding requirements are complete, including the New Grad Residency (where applicable).

Skills

  • Excellent communication and relationship-building skills.
  • Ability to provide compassionate, patient-centered care.
  • Strong organizational and time management skills.

Pay

  • This is an exempt, salaried role.

Schedule
