About the role
Registered Nurse (RN) Hospice, providing specialized end-of-life care in patients' homes or facilities.
Responsibilities
- Assess patient needs
- Develop and implement personalized care plans
- Collaborate with healthcare teams
- Promote culturally sensitive care
- Evaluate patient condition changes
- Communicate updates
- Support patients and families emotionally, spiritually, and physically
- Ensure quality and evidence-based care while maintaining safety standards
- Support community travel
- Participate in a set schedule with weekend options
- Contribute to holistic patient care through interdisciplinary teamwork
Qualifications
- Nursing diploma or associate's degree
- RN license
- Valid driver’s license
- Less than 2 years experience preferred
- Bachelor’s degree and 2-5 years of relevant experience preferred
- Prior hospice or admissions experience advantageous
