$35.71 – $70.74 per hour

About the role

The Homecare Admission Nurse reports to the Nursing Supervisor and ensures quality and safe delivery of nursing and home health services. Coordinates provision of home health care services that reflect the agency philosophy and standards of home health and nursing care of assigned patients. The primary function is to admit patients to service, including medication reconciliation and completion of the OASIS data set. An Admission Nurse typically completes 2–3 admissions per day and may also perform revisits as needed to meet agency productivity.

Qualifications

• Education: Bachelor's or Master's degree in Nursing or equivalent experience preferred
• Experience: Minimum one year in Home Health Care; minimum one year as a Home Health Admission Nurse; leadership skills and abilities strongly preferred
• Licensure: Current license to practice as a registered professional Nurse in Massachusetts
