About the role

Deliver skilled nursing care to patients in their homes throughout assigned Pittsburgh-area communities.

Develop and manage plans of care in collaboration with physicians and interdisciplinary partners.

Provide patient and family education that promotes independence and recovery.

Complete required documentation (OASIS, visit notes, orders) electronically using a provided laptop.

Contribute to high-quality, patient-centered outcomes while maintaining regulatory standards.

Responsibilities

Requirements

  • Graduate of an accredited school of professional nursing.
  • Active RN license (Pennsylvania or compact state eligibility).
  • Valid driver’s license, auto insurance, and required clearances.
  • CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.
