We are seeking an experienced and compassionate Registered Nurse (RN) to join our growing Home Health team serving patients throughout Albuquerque and Rio Rancho. This position is ideal for an RN who has a strong background in home health, is comfortable working independently in the field, and has hands-on experience completing Start of Care (SOC) assessments and OASIS documentation. Responsibilities
- Complete comprehensive Start of Care (SOC) assessments
- Complete accurate and timely OASIS documentation
- Perform skilled nursing assessments and visits
- Develop and update individualized plans of care
- Monitor changes in patient condition and communicate with physicians and the interdisciplinary care team
- Provide patient and family education
- Coordinate care with therapists, physicians, caregivers, and other healthcare professionals
- Maintain accurate, timely, and compliant clinical documentation
- Help identify changes in condition and opportunities to improve patient outcomes and prevent unnecessary hospitalization
Requirements
- Active New Mexico Registered Nurse (RN) license in good standing
- Minimum 2 years of total nursing experience
- Minimum 1 year of Home Health nursing experience
