This position is ideal for an RN who enjoys providing one-on-one patient care and helping patients maintain their health, safety, and independence at home. Key Duties Conduct comprehensive nursing assessments during home visitsDeliver skilled nursing services according to physician orders and established care plansMonitor vital signs, symptoms, medications, and overall patient progressProvide wound care, medication management, and other skilled nursing services as appropriateEducate patients and caregivers on disease management and home careIdentify and report significant changes in patient conditionCoordinate with physicians, therapists, case managers, and other members of the healthcare teamComplete clinical documentation accurately and within required timelinesMaintain compliance with applicable clinical and safety standards Required Qualifications Active RN license or eligibility for appropriate Pennsylvania licensureGraduate of an accredited School of NursingNCLEX-RN passedCurrent BLS/CPR certificationStrong clinical assessment and communication skillsAbility to manage patient visits independentlyPrevious nursing experience preferred; Home Health experience is a plus Preferred Bachelor of Science in Nursing (BSN)Home Health, Med-Surg, Rehabilitation, or Skilled Nursing experienceExperience with electronic medical records and home health documentation At Srimatrix Health, we're dedicated to connecting talents with opportunities that transform lives.
