Registered Nurse / RN (Home Health)

Responsibilities
- Thoroughly assess patients and their families to identify home care needs.
- Provide comprehensive physical assessments and document patient histories.
- Develop and update individualized care plans with active patient and family participation.
- Identify and address discharge planning needs before patient discharge.
- Offer counseling to patients and families on nursing and related health needs.
Requirements
- Current and valid state RN license.
- Minimum of 1 year of recent acute care experience in an institutional setting.
- Basic Life Support (BLS) certification, including hands-on CPR.
- Valid driver's license and auto insurance.
- A minimum of an Associate's or Bachelor’s degree in Nursing, with 1 year of home health experience preferred.
Qualifications
Experience in an institutional setting and a current RN license are essential. Additional qualifications include a valid driver's license, auto insurance, and a degree in Nursing with relevant experience.
Skills
Strong assessment and documentation skills, excellent communication, and the ability to work independently and as part of a team are required.
