Registered Nurse / RN, Home Health

Position Type: Full-Time

Coverage Area: Jasper & Newton Counties, MS

Pay

$71,000‑$79,000 per year (estimated). This position is paid on a per‑point basis.

Schedule

Monday‑Friday 8:00 am – 4:30 pm. On call: 1 week every 5‑7 weeks.

Qualifications

• Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
• Minimum of one year of experience as an RN.
• Current CPR certification required.

Responsibilities

• Assess the status of nursing needs of assigned patients and manage the patient’s care plan.
• Complete initial patient assessment upon admission, including appropriate patient history.
• Select appropriate nursing diagnosis.
• Assess all medicines to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contraindicated medications.
• Establish realistic, measurable, observable goals consistent with the patient’s diagnosis.
• Inform the physician of the results of the assessment and recommended clinical interventions.
• Implement appropriate nursing interventions consistent with the patient's diagnosis and established goals and within the scope of the Nurse Practice Act.
• Re‑evaluate patients' nursing and other clinical needs as required.
• Document and submit the assessment, progress, and discharge notes and other required paperwork in a timely fashion as required by the agency.
• Inform the physician and other personnel of changes in the patient's condition and needs.
• Direct staff nurses, LPNs, and CHHAs in delivering the individual patient’s plan of care, as well as identifying interdisciplinary needs and coordinating allied health clinicians.
