Registered Nurse/ RN, Home Health

About the Role

RN Case Manager, Home Health based in the Austin office, serving Bastrop, Del Valle, Lockhart, and Smithville. Full‑time position (not remote/virtual). Responsible for managing patients’ care plans from admission through discharge, directing staff nurses, LPNs, and CHHAs, and coordinating interdisciplinary care.

Responsibilities

• Assess the status of nursing needs of assigned patients and overall manage the patient’s care plan.
• Complete initial patient assessment upon admission, including appropriate patient history.
• Select appropriate nursing diagnosis.
• Assess all medicines to identify ineffective drug therapy, adverse reactions, significant side effects, drug allergies, and contraindicated medications.
• Establish realistic, measurable, observable goals consistent with the patient’s diagnosis.
• Inform the physician of assessment results and recommended clinical interventions.
• Implement appropriate nursing interventions consistent with the patient’s diagnosis, established goals, and within the scope of the Nurse Practice Act.
• Re‑evaluate patients’ nursing and other clinical needs as required.
• Document and submit assessment, progress, and discharge notes and other required paperwork timely as required by the agency.
• Inform the physician and other personnel of changes in the patient’s condition and needs.

Qualifications

• Graduate from an approved school of professional nursing.
• Currently licensed to practice as a registered nurse in the state of agency operation.
• Minimum of one year of experience as an RN.
• Current CPR certification required.

Compensation and Schedule

Salary range: $91,000–$112,000 (estimated annual compensation). Schedule: Monday‑Friday, 8 am–5 pm. On‑call required. Compensation is paid on a per‑point basis.
