Registered Nurse (RN) - Home Health - $35-51 per hour

About the role

We are seeking a compassionate Registered Nurse (RN) for a home health role focused on delivering patient-centered care in patients' homes and via telehealth.

Responsibilities

• Conducting in-home assessments, safety evaluations, and physical examinations.
• Performing clinical interventions such as medication administration, wound care, point-of-care testing, and other nursing procedures.
• Collaborating with physicians, nurse practitioners, and interdisciplinary teams to develop and implement personalized care plans.
• Providing patient and caregiver education, supporting care transitions, and addressing health concerns.
• Maintaining accurate documentation, supporting quality initiatives, and participating in team meetings.
• Supporting community relationships and ensuring compliance with regulatory standards.

Qualifications

• Active RN licensure with 3+ years of direct patient care experience.
• CPR certification; telehealth and home care experience preferred.
• Proficiency in electronic health records, Microsoft Office, and telehealth platforms.

Skills

• Strong communication, organizational, and decision-making skills.
• Ability to work independently, handle administrative tasks, and travel locally for patient visits.
