Registered Nurse (RN) - Home Health - $35-51 per hour

About the role

We are seeking a compassionate Registered Nurse (RN) for a Home Health role focused on delivering patient-centered care in Charleston, West Virginia.

Responsibilities

• Performing in-home patient visits, assessments, and care coordination under the supervision of licensed providers.
• Conducting home safety evaluations, documenting findings, and delivering clinical interventions such as medication administration, wound care, and point-of-care testing.
• Providing patient and caregiver education, supporting care transitions, and addressing health concerns.
• Collaborating with interdisciplinary teams to develop and implement personalized care plans.
• Maintaining accurate records, adhering to regulatory standards, and participating in team meetings.
• Building relationships with healthcare providers and community resources to enhance patient support.

Qualifications

• Active RN license, with preferred home care, hospice, palliative, or geriatrics experience.
• Minimum of 3 years clinical experience in a patient-facing role.
• CPR certification and valid driver’s license with auto insurance.
• Proficiency in electronic health records, telehealth platforms, and Microsoft Office.

Skills

• Ability to conduct in-home visits and telehealth consultations.
• Strong communication, organizational, and decision-making skills.
• Ability to work independently, manage administrative tasks, and engage with diverse populations.
• Physical ability to lift up to 20-50 lbs, stand for extended periods, and drive locally.
