Position Details
Location: Austin South Office – Coverage Area: Del Valle, Dale, Bastrop, Lockhart
Position Type: Full‑Time (Remote/Virtual: No)
Responsibilities
- Assess the status of nursing needs of assigned patients and manage the patient’s care plan from admission through discharge.
- Complete initial patient assessment upon admission, including appropriate patient history.
- Select appropriate nursing diagnoses.
- Assess all medicines to identify possible ineffective drug therapy, adverse reactions, significant side effects, drug allergies, and contraindicated medications.
- Establish realistic, measurable, observable goals consistent with the patient’s diagnosis.
- Inform the physician of assessment results and recommended clinical interventions.
- Implement appropriate nursing interventions consistent with the patient’s diagnosis and established goals, within the scope of the Nurse Practice Act.
- Re‑evaluate patients' nursing and other clinical needs as required.
- Document and submit assessment, progress, discharge notes, and other required paperwork in a timely fashion.
- Inform the physician and other personnel of changes in the patient’s condition and needs.
- Direct staff nurses, LPNs, and CHHAs in delivering the individual patient’s plan of care and coordinate interdisciplinary and allied health services.
Qualifications
- Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
- Minimum of one year of experience as an RN (minimum of 6 months of Home Health experience required).
- Current CPR certification.

Pay
Salary range: $83,000 – $102,000 per year (estimated annual compensation; position paid on a per‑point basis).
Schedule
Monday‑Friday, 8 am – 5 pm. On‑call duties required.
