Registered Nurse (RN) - Geriatrics / PACE

Overview

Join a community-based senior care program focused on helping older adults stay safe, supported and independent. This RN role blends clinic-based care with occasional home/facility visits and works closely with an interdisciplinary team in a collaborative, patient-centered environment.

Responsibilities

• Provide Skilled Nursing Care and administer treatments per provider orders.
• Assess participants and support individualized care planning.
• Educate and counsel participants and families.
• Care for coordination of care and communication with the interdisciplinary team.
• Complete documentation and required assessments.
• Perform local/home/clinic Visits as needed (mileage reimbursement provided).

Qualifications

• Active, unencumbered Delaware RN License.
• CPR certification (Active).
• 2+ years RN experience working with Geriatric patients.
• Valid Driver's License (DE) + reliable personal vehicle/insurance.
• Strong communication, organization, and teamwork skills.
• B.S.N. preferred.

On-Call

• Minimum, remote, telephonic only.

Salary/Compensation

$84,000 - $105,000 / year based on experience.
