Registered Nurse/RN Full-Time

About the Role

This position requires substantial specialized judgment and skills within the framework of behavioral health treatment. Supervision is received from the Medical Director, Associate Medical Director, Behavioral Health Center Directors, and the Vice President for Behavioral Health.

Qualifications

• Completion of a course of study approved by the New York State Education Department as qualifying for Registered Professional Nurse (RN).
• Possession of a New York State Registered Nurse’s License at time of application.
• Must be authorized to work in the United States without work sponsorship.
• Must be able to stand and walk for prolonged periods of time.

Responsibilities

Behavioral Health (BH)

• Assess behavioral health client nursing care needs.
• Provide skilled nursing care and health monitoring services to clients.
• Coordinate client care with treatment team members and health care professionals in the community.
• Counsel and guide clients and families in the prevention of illness and in the attainment and maintenance of maximum health.
• Complete prior authorization medication forms and work with insurance/managed care organizations to advocate for medication coverage.
• Plan, schedule, and coordinate client appointments for injection medications, collection of vital signs with administration of psychotropic injection (LAI).
• Maintain essential nursing case records and reports.
• Develop and oversee procedures related to nursing care.
• Monitor Medication Storage Cabinets and inventory.
• Manage nursing supplies and medical hazardous waste.
• Provide education to clients and staff on health-related topics.
• Complete health screening forms for new clients and annually.
• Develop and facilitate groups specific to behavioral health and wellness.
• Other duties as assigned.

Medication-Assisted Treatment (MAT)

• Provide care coordination services for patients eligible for the MAT program.
• Screen patients interested in MAT for fit and arrange initial appointments.
• Lead client intake process including reviewing the program and ensuring completion of enrollment paperwork.
• Provide patient education (e.g., risks and benefits of Suboxone or Sublocade, symptom management/withdrawal, understanding pain) and triage concerns as they arise.
• Thoroughly assess and document patient’s signs and symptoms.
• Accurately and appropriately document client behaviors and self-reports.
• Monitor lab tests, e.g., urine toxicity, blood tests.
• Administer evidence-based outcome measurements such as the Clinical Opiate Withdrawal Scale (COWS) and the Clinical Institute Narcotic Assessment.
• Assist with patients’ stressors and resources as they arise.
• Obtain prior authorizations and coordinate with pharmacies for medications.
• Enhance patient follow-up and program adherence.
• Consult and coordinate with MAT prescribers and behavioral health providers.
• Communicate patient status changes to the Care Team—teaming with MAT provider, primary care provider, behavioral health provider, nurse, pharmacists, and other internal and external supports.
• Complete external referrals for medical needs by MAT patients.
• Maintain DEA list of active patients.
• Promote MAT program to community members and partners.
• Develop and maintain partnerships with community members and organizations.
• Assist patients in meeting the goals of their particular treatment tier while they work towards program graduation.
• Other duties as assigned.
